• SOCIAL SECURITY INCOME
  • Copy of Social Security Check, or
  • Latest bank statement showing direct deposit, or
  • Notice from Social Security office with current amount

   

• PENSIONS
  • Previous yearís statement of pensions earned

   

• STOCK DIVIDENDS
  • Interest earned in previous year stock dividends, and cash value of all stocks owned

   

• SAVINGS ACCOUNT
  • Current balance, and interest earned in the previous year from all bank accounts

   

• CHECKING ACCOUNTS
  • Bank statements from the last six (6) months

   

• DISPOSED ASSETS
  • Declare all assets that were disposed or transferred within the last two years

   

• EMPLOYMENT
  • Current earnings from employment (if applicable)

   

• MEDICAL EXPENSES
  • Latest premium notices showing amount due for health insurance
  • receipts of recurring medical expenses not covered by insurance (if applicable)

   

• SOCIAL SECURITY CARD
  • The Department of Housing and Urban Development requires that we have a copy of your Social Security card on file.
